Acalasia: diagnóstico e terapêutica

Resumo

A acalasia é um distúrbio motor do esôfago caracterizado pelo relaxamento inadequado do esfíncter esofágico inferior e perda da peristalse organizada no corpo esofágico. A clínica é variável, porém a maior parte dos pacientes apresenta disfagia para sólidos e líquidos, regurgitação, dor torácica ocasional e perda ponderal. O diagnóstico é feito através de uma clínica compatível, além da manometria esofágica de alta resolução, atual método padrão ouro. Atualmente dispomos de diferentes tratamentos, desde a dilatação pneumática da cárdia até técnicas cirúrgicas como miotomia a Heller e a miotomia endoscópica peroral, cuja escolha deve ser individualizada para cada paciente.
